Strengthening Diabetes Awareness and Access in Our Community
Over the past several months, the Culmore Clinic team has continued advancing the CareFirst Diabetes Project with a focus on education and meaningful patient engagement. Through close collaboration with our clinical partners, community organizations, and bilingual outreach teams, we are helping patients understand their risk and connect them with supportive resources.
Our team coordinated diabetes education appointments, supported front‑office staff with patient navigation, and provided culturally responsive materials to help our patients stay informed and empowered in their care journey.
There have been many recent highlights of the project. Some examples include our work delivering bilingual diabetes education and risk‑awareness materials to patients, supporting clinic staff with appointment coordination and patient follow up, encouraging community members to make preventative lifestyle changes and strengthening partnerships with our providers.
These activities reflect our commitment and as we move into the next phase, we will continue building on this momentum with expanded outreach, additional educational sessions, and even deeper ties to the community.
