Diabetes Awareness Month:  How We're "Turning the Curve"  Plus Featured on Telemundo44!

November has been Diabetes Awareness Month! We are so grateful for all of our partners involved in Turning the Curve for Patients Living with Prediabetes, Diabetes and Other Metabolic Illnesses recently featured on Telemundo44! The goal of this pilot is to develop an integrated, patient-centered healthcare program aimed at improving the health outcomes for our patients by engaging them in interventions to help them manage chronic illnesses.

Culmore Clinic featured on Telemundo44.

Initially launched with seed money from Kaiser Permanente, this three-year pilot program is possible thanks to a generous partnership with Islamic Relief USA, a nonprofit humanitarian agency and member of the Islamic Relief Worldwide group of organizations. We are so grateful for their support!

United Healthcare also provides Culmore Clinic patients in the program with the tools they need for success in managing their illness. They include portion control containers, wrist and ankle weights, as well as exercise bands for patients to take home and continue to practice the evidence-based interventions they are learning about through our Diabetes Management Workshops, a seven-month series of sessions led with educators and pharmacists from NovaScripts Central.
